List your Business business / category.
List the shareholders and percentage share allocations.

NOTES: If you are applying offshore you need to have a local or friend who can sign a letter at the Notary on your behalf.
If you are applying in Bali, you need to be on a B211A first to apply without having to exit Indonesia.
